***This form will also be emailed to you upon scheduling***
*** Please only complete after scheduling your initial appointment***
-This intake form is optional, however, if it is not completed, we will need to spend extra time at the appointment talking over important factors of your health history, goals for treatment and preferences. This will take away from your time on the table. Please choose the option that feels most accessible to you.
-At the appointment, you will sign a consent to treat for yourself or your child.
-A copy of the client bill of rights can be found here
-If there are any documents, lab results or notes from your doctor or other practitioners you would like me to be aware of prior to your treatment please bring them with you to your appointment.
Craniosacral Pricing:
Unfortunately, we cannot accept insurance and these services are all self pay out of pocket.
We can accept card, cash, venmo, zelle and apple pay
As always, tips are not accepted and tax will be added at checkout to all credit card transactions.
Integrative Body Work Pricing
For those who are in need of deeper support to ease tension and calm the nervous system.
As always, I do not accept tips as this is a health and healing practice, not a luxury.
Tax will be added at checkout unless you have a referral from a medical professional for massage.
