-This intake form is optional, however, if it is not completed, we will need to spend extra time at the appointment talking over important factors of your health history, goals for treatment and preferences. This will take away from your time on the table. Please choose the option that feels most accessible to you.
-At the appointment, you will sign a consent to treat for yourself or your child.
-A copy of the client bill of rights can be found here
-If there are any documents, lab results or notes from your doctor or other practitioners you would like me to be aware of prior to your treatment please bring them with you to your appointment.
